It's a professionally created 'expansion' type engine/expansion-pack, with new monsters, weapons and powerups- all of which VERY professionally designed and fitting into the Quake theme and gameplay.

They only made a couple of maps (most distinctively remakes of the start map and the first 2 Ep1 maps (with a few testmaps to go with it).
However, lots of awesome Quoth levels are available separately (Breakfast at Twilight by Hrimfaxi being the very best)

There are some popular mod/expansions like Insomnia, Warpspasm, Beyond Belief, Nehahra, Zerosteror- Testament to the Destroyer you might be interested in too.

Otherwise most of the best productions are stand-alone maps- you might like to check out Kona's stuff, there's some good stuff in there.
