I was going to mention that many custom maps don't have a cd track associated with them, so I don't think a cd (physical or virtual) will play. The original maps and the two official mission packs' maps are set up as such. However, if the option is not selectable, then there's probably another problem.
Quake prints a bunch of lines in the console when it's first opened, usually displaying hardware detection and such. If you're lucky, it'll say something about sound or a cd drive.
Though, if you just want an entire cd to play in the background, I'd say you might as well use a seperate cd player from Quake.
