Is there a special way to complete this setting? I've played through the game twice on this setting and after I complete chapter 4, the other chapters open back up as if they were never completed. What am I doing wrong?

that's funny that you said this I have been having the same issue after completing all four levels the first three open back up and as far as I can remember you would beat quake and it would give you the message at the end.

IIRC this is an issue of older darkplaces builds from 2011 and before.
update to any newer darkplaces build from 2013 or 2014, which dont have this bug

i recommended the 20140304 darkplaces build, its the latest stable version
darkplaces 20140304 Universal build for Windows/Linux/Mac
darkplaces 20140304 Build for 64bit Windows only

      .
are you curious about what all there is out there in terms of HD content for quake?
      > then make sure to check out my 'definitive' HD replacement content thread! <
everything that is out there for quake and both mission-packs, compiled into one massive thread

I've not used any mods, it's vanilla in non GL mode

Yes, it is a bug (an old one).
You will find how and why it happens (and how to get over it) here
Has been discussed here and here as well.

It seems to happen with WinQuake (this is what bear is using, as it comes with Steam) and DosQuake. GLQuake seems to have fixed it already. Chances are high that advanced engines from nowadays fixed it as well, as the source is gpl. If not, the engine devs are too lazy

I can't play GL Quake because the game crashes when I try to change the resolution. =( The default is too low and my tv cuts off the hud.
